Notes:
- Student/staff must submit this form to the Admissions Office within 7 working days of the cause of the grievance/complaint. The school will take 21 working days to investigate and resolve the matter. Read this page for Grievance Process.
- Feedback from an anonymous person will not be investigated and addressed.
- Your personal information is only used by the school to address the feedback you have provided. The school will not disclose your personal information to third party without your consent, unless required or permitted by law. Your feedback will be filed for 3 years and then destroyed at the end of the third year.
